What to bring
If you are the patient: your KRC card, and your PWD or senior citizen ID.
If you are family: the patient's KRC card, and your own valid ID. Your name needs to be on the patient's signup form. That is how a partner knows you belong to the family, so write everyone down on the day you sign up.
